Contact messages
The contact form sends your name, email, optional phone, topic and message to the administration by email at contact@spartdental.com and to its Telegram chat. The email is sent through the hosting provider’s SMTP service. This form does not use FormSubmit.
A private server queue retains the message until both delivery services accept it. Its contents are then removed from that queue. Messages remain in the clinic’s mailbox and Telegram chat under those services’ terms and the clinic’s retention practices. Technical delivery references and hashed rate-limit data may remain temporarily on the server. No form fields are saved in browser storage.
Appointment requests
The appointment form sends your contact details, chosen treatment, preferred dentist, date and language, whether you are visiting from abroad, and your consent using the same submission process as the contact form. No form fields are saved in browser storage.
Sending your request
The send button submits the form directly. You do not need to send a separate email. An appointment is confirmed only after you agree its date and time with the administration.
Medical and personal details
Use the form only for basic contact and appointment preferences. Do not include medical records, identification documents, payment card details, or sensitive health information. Ask the clinic how to share anything needed for your care.
External services
The Facebook video player loads only when you choose a video. Facebook processes that interaction under its own privacy terms. You can also open videos and Google reviews directly on the respective services.
This website embeds a Google Map and links to Instagram, Facebook, and source publications. The map loads content directly from Google; these services have their own privacy practices. The hosting service may process technical request data needed to deliver and secure the website.
